• Basic Plan
  Best for those under 40, or anyone looking to keep things minimal with medications.

• Advanced Plan
  Our most popular option. Ideal if you want a broader range of medication options, want to avoid cancellation fees, and prefer fewer required visits.

• Comprehensive Plan
  Perfect for those seeking the most in-depth longevity care, priority scheduling, longer visits, and access to specialty medications not available in lower tiers.

Your out-of-pocket cost per visit depends on your insurance plan—especially your deductible. Here’s what to expect:

Low-deductible plans:

• Most patients pay $20–$50 per visit.

High-deductible plans:

• Costs can range from $150–$300 per visit until your deductible is met.

Important note: Your insurance company sets these rates, not us.

• If you have a high deductible, it’s often more cost-effective to enroll in one of our out-of-pocket membership plans, which do not have copays or no show or cancellation fees.

Visits are typically scheduled for 30 minutes, unless you're on the Comprehensive plan. In that case your visits can be 30, 60 or 90 minutes long.

Because longevity care involves multiple areas of health—we build your plan step by step over a series of visits. It's not all packed into one session.

Want to make the most of your time?
Share as much as possible in pre-visit forms and questionnaires to enable your provider to be efficient during your appointments.

Most of our patients do not have to worry about such fees.

However if you're on the Basic plan via insurance, you’ll be charged a $200 no-show or late cancellation fee if you

• Cancel less than 48 hours before your visit

  OR

• Arrive more than 10 minutes late

Why the fee?
Our Basic insurance plan relies entirely on insurance billing. Missed or shortened visits mean we can’t cover the cost of your care.
Patients on Advanced and Comprehensive plans (or the Basic plan outside of insurance) don’t pay this fee because their monthly membership helps cover scheduling flexibility.

Want to avoid future fees?
If you’re charged a $200 fee, you can apply it toward upgrading to a monthly membership plan—so you won’t be charged for no-shows or late cancellations again.

We use visit minimums to ensure you're staying engaged with your care—and to keep costs low while accepting insurance.

• Basic Plan: Requires an average of 1 visit per month

• Advanced Plan: Requires an average of 1 visit every 2 months

We check your visit average at the end of each 1-month (Basic) or 2-month (Advanced) interval.
If you're below your required average (e.g. only 5 visits by the end of 6 months for a Basic plan or only 2 visits at the end of 6 months for an Advanced plan), a $200 fee will be charged for that period.

Good news: Most patients build up a buffer early on. The initial longevity assessment typically involves 2–4 visits in the first month, which often keeps you ahead of the required pace for months to come.

We recommend committing to at least 12 months to see the best results from your longevity plan—but we understand that some patients may need to exit early.

Here’s how early termination works:

Basic Plan:

• If you’ve completed all required visits, the $300 termination fee is waived.

• If required visits haven’t been met, a $300 early termination fee applies to help cover care already provided.

Advanced & Comprehensive Plans:

• You can either pay the $300 early termination fee

  OR

• Pay the remaining monthly membership fees for the year—whichever is less.

We aim to keep your options flexible while ensuring continuity and sustainability of care.

Basic Plan:

• Messaging is limited to administrative staff only.

• Messages will be relayed to your provider, but clinical concerns must be addressed during visits.

• Messaging is typically limited to scheduling or pharmacy-related updates.

Advanced & Comprehensive Plans:

• You can message directly with your provider.

• This allows for ongoing support between visits—including:

  • Side effect management

  • Medication adjustments

  • Supplement questions

  • Interpretation of labs, tests, or wearable data

Priority Booking:

Comprehensive Plan Only:

• Access to same-day and next-day appointments

• Ability to book multiple or recurring appointments

Other Plans: Appointments must be booked at least 2 days in advance, and only one appointment can be scheduled at a time

Extended Visits:

Comprehensive Plan Only: Option for 60 or 90 minute visits for more in-depth support

Other Plans: Limited to only 30 minute visits
